Lots of lose skin after 200 lbs weight loss What can be done to fix this?

Allow me to answer your question as best i can based upon your photos. However, please keep in mind that an in-person, or at least a virtual, consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made.

Buttock “sagging” and/or loose skin is usually a combination of loss of volume or deflation (after weight loss, liposuction, or post-BBL) and/or the effect of aging and gravity. So,  I personally first treat patients like you by restoring volume to the buttock with a buttock implant. This will create a more desirable smooth and rounded perky buttock profile. If not adding volume with an implant, the buttock will most certainly always appear more flat if just a buttock lift from above is performed. On the other hand, an “autologous buttock lift” usually provides very little volume and projection, and almost nothing to the lower 2/3rds of the buttock.

A lower buttock tuck is usually then performed at six months following implants. Coincidentally, a lower buttock tuck also doubles as a thigh lift, removing loose wrinkled skin from this area as well. In this sequence, the implants have fully settled into final position so that any remaining loose skin can be most accurately identified and removed with the lower buttock tuck. Any “non-surgical” treatment or device as well as exercise, unfortunately, will have little to no effect on this problem area. Furthermore, fat transfer and “fillers” perform very poorly in this area and usually result in just a waste of hard earned money.

Please note, because an extremely few number of board certified plastic surgeons specialize in buttock implants, you may have to travel away from your local area to seek consultation with this particular type of surgeon.  Getting it done right the first time around is not only the best chance for success, but also will save you from a lot of headaches and money in the future. Glad to help.

You need a posterior thigh lift.  A curvilinear incision is used posteriorly to elevate the buttocks higher.  At the time of the surgery, excess skin and fat can be used as a flap to rotate in and give more projection if desired.

Thank you for your question and photo. You would need a buttock lift, which normally is part of a circumferential body lift. I assume you do have sagging skin over the abdomen as well. The posterior thighs will not be improved with this surgery. You have irregularities due to cellulite and deep fibrous attachments. These are best treated with the GoldIncision technique, separately under local anesthesia. More than one session is needed to achieve a nice result. The best procedure for what you describe is going to be a thigh lift.
It will give some improvement, but will probably still leave you with some skin wrinkles or apparent skin laxity.
When we look at skin issues from age and or weight loss there are two variables that need to be considered.
One is skin laxity, which is usually related to weight loss or potentially pregnancies in regards to the abdomen.
The other is skin elasticity which we lose with age.
Individuals who have great skin elasticity will do much better with procedures that treat skin laxity.
In other words, young people get better results from skin surgery than those who are on the older side.
Likewise, those with good elasticity may not show as much laxity from weight loss etc.
A thigh lift has some drawbacks and doesn’t have the greatest patient satisfaction compared to other procedures.
It can, however, be a good option.
I suggest you consult with several plastic surgeons in your area.
Bring pictures of yourself to use as reference during the consultation.
 During each consultation, ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of previous patients who had similar body characteristics for whatever procedure they are recommending.Best,
